Housing instability is defined by the HL7 Gravity Project as currently consistently housed but experiencing any of the following circumstances in the last 12 months: behind on rent or mortgage, 3 or more relocations, housing cost burden, homelessness, or risk of eviction. LOINC is used for Social Determinants of Health (SDOH) observations (assessment instrument panels, questions, and answers.) SNOMED CT U.S. Edition is used for SDOH observations, conditions, goals, diagnoses, procedures and interventions. ICD-10-CM is used for SDOH diagnoses. CPT and HCPCS Level II are used for SDOH procedures and interventions.
