SDOH Problems/Health Concerns represent problems and diagnoses identified by the Gravity Project and as determined by screening assessments of a SDOH-related risk. These are represented by SNOMED CT U.S. Edition and ICD-10-CM codes that have been identified through the Gravity Project community consensus voting process. The Gravity Project established and maintains current value sets which represent SDOH Conditions and Diagnoses within many SDOH domains, such as Food, Housing, and Transportation Security. The United States Core Data for Interoperability version 3 (USCDI v3) includes SDOH Problems/Health Concerns as a data element used to capture and exchange data on a patient's SDOH risks and needs. The value sets included on this page are intended to guide development of the ability to capture this type of data in health IT. A white paper developed by National Council for Prescription Drug Programs (NCPDP), Collecting and Exchanging Social Determinants of Health Data in the Pharmacy, may assist in the usage of ICD-10-CM and SNOMED CT US Edition codes within the NCPDP SCRIPT and Telecommunication Standards, as well as the Pharmacist eCare Plan. NCPDP Comment

NCPDP recommends that ONC add to the Limitations, Dependencies, and Preconditions for Consideration section: NCPDP supports the use of ICD-10-CM and SNOMED CT US Edition codes to represent SDOH Conditions and Diagnoses. NCPDP produced a white paper, Collecting and Exchanging Social Determinants of Health Data in the Pharmacy, to assist in the usage of these codes within the NCPDP SCRIPT and Telecommunication Standards, as well as the Pharmacist eCare Plan.

PHIT supports ONC’s recommendation for using SNOMED CT U.S. Edition and ICD-10-CM to represent SDOH conditions and diagnoses. PHIT also recommends reviewing NCPDP’s white paper, “Collecting and Exchanging Social Determinants of Health Data in Pharmacy,” which was produced to assist in the usage of these codes within NCPDP SCRIPT and Telecommunications Standards and the Pharmacist eCare Plan.
