Dr. O’Malley is a geriatrician active in teaching and patient care while conducting research and implementing programs on transitions in care and post-discharge patient safety. He started the Clinical TransitionsCommittee within Partners HealthCare in 2002 and has co-chaired it since. This work led to participation on the UHC Transitioning Care Steering Committee in 2006, the Joint Commission Center (TJC) for Transforming Health Care “Improving Hand-Off Communication Collaborative 2009-2011 and implementation of the TJC programs as part of Partners Patient Safety.  Until 2014 he was an Associate Medical Director within Partners Continuing Care, the non-acute services organization within Partners, and the Medical Director for Non Acute Care Services for Partners HealthCare System where he oversaw communication among post-acute sites of care and implemented systems to standardize the process of care transitions making them more reliable and safer.  Dr. O’Malley’s work has been at the junction of long term care, transitions of care, longitudinal coordination of care, quality measurement, and electronic health information exchange with an emphasis on person-centered goals and priorities.
