ONC has just released a <a href=\"https:\/\/healthit.gov\/wp-content\/uploads\/2017\/09\/key_considerations_for_hios_stage_2_toc_final_0.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">new brief<\/a> that provides support.<\/p>\n\n\n\n<p>This is a part of one of the ONC Health Information Exchange (HIE) team\u2019s goals \u2013 to provide support and direction to grantees.&nbsp; Recently, ONC released comprehensive interoperability training modules that are geared toward multiple stakeholder audiences and provide guidance from the elementary level to detailed technical guidance for experts involved in implementation.&nbsp; <\/p>\n\n\n\n<p>The modules cover Stage 2 Meaningful Use (MU) topics that are available on <a href=\"https:\/\/healthit.gov\/health-it-basics\/interoperability-training-courses\" target=\"_blank\" rel=\"noopener\">HealthIT.gov<\/a>.&nbsp; The training topics include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>introduction<\/li>\n\n\n\n<li>transitions of care exchange<\/li>\n\n\n\n<li>lab exchange<\/li>\n\n\n\n<li>public health exchange (coming soon)<\/li>\n\n\n\n<li>patient view-download-transmit capability (coming soon)<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-onc-brief-on-stage-2-mu-transition-of-care-measures\"><b>ONC brief on Stage 2 MU Transition of Care measures<\/b><\/h2>\n\n\n\n<p>To provide more targeted and explicit guidelines to our state HIE program grantees, we are issuing a<span style=\"color: #000000\"> <a href=\"https:\/\/healthit.gov\/wp-content\/uploads\/2017\/09\/key_considerations_for_hios_stage_2_toc_final_0.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">new brief <\/a><\/span>that provides Health Information Service Providers (HISPs) and Health Information Organizations (HIOs) with available options for supporting the Stage 2 MU Transition of Care measures.<\/p>\n\n\n\n<p>The brief lays out the various ways HISPs and HIOs can support eligible providers and hospitals for Stage 2 Transition of Care exchange, and will assist HISPs and HIOs as they build their strategic and operational roadmaps for the next few years.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-meeting-stage-2-mu-transition-of-care-measures\"><b>Meeting Stage 2 MU Transition of Care measures<\/b><\/h2>\n\n\n\n<p>Eligible providers and hospitals have a number of options to choose from in order to meet the Transition of Care measures, such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>working with an HIO, or<\/li>\n\n\n\n<li>electronically sending a summary of care document for 10% of transitions of care.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-achieving-cms-ehr-incentive-program-measures\"><b>Achieving CMS EHR Incentive Program measures<\/b><\/h2>\n\n\n\n<p>The brief also provides best practice recommendations to ensure the CMS EHR Incentive Program audit measures can be achieved by eligible providers.&nbsp; While CMS will issue official guidance in the future, we have tried to build in a baseline of anticipated functionality for planning purposes to support these measures.<\/p>\n\n\n\n<p>Please leave any thoughts or questions you have in the comment section below.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Are you looking for more support on how to meet the transition of care measure [&hellip;]<\/p>\n","protected":false},"author":619,"featured_media":0,"comment_status":"open","ping_status":"open","sticky":false,"template":"","format":"standard","meta":{"_acf_changed":false,"_selected_menu":"","_show_breadcrumbs":"true","_blog_show_featured_image":false,"footnotes":""},"categories":[24],"archived-category":[705,713],"featured":[],"topics":[439,440],"class_list":["post-4704","post","type-post","status-publish","format-standard","hentry","category-health-information-exchange-2","archived-category-meaningful-use","archived-category-state-hie","topics-historical-programs","topics-meaningful-use-macra"],"acf":{"blog_authors":[{"blog_author_profiles":"198501"}],"hp_news_hide":false},"yoast_head":"<!-- This site is optimized with the Yoast SEO Premium plugin v24.3 (Yoast SEO v24.8.1) - https:\/\/yoast.com\/wordpress\/plugins\/seo\/ -->\n<title>New Tips for Meeting the Stage 2 Meaningful Use Transition of Care Measure<\/title>\n<meta name=\"description\" content=\"ONC releases new brief on how to meet the Transition of Care measure that is required to meet Stage 2 meaningful use.
