{"id":3305,"date":"2012-07-18T10:23:42","date_gmt":"2012-07-18T14:23:42","guid":{"rendered":"http:\/\/www.healthit.gov\/buzz-blog\/?p=3305"},"modified":"2026-01-05T21:55:25","modified_gmt":"2026-01-05T21:55:25","slug":"health-information-exchange-work","status":"publish","type":"post","link":"https:\/\/healthit.gov\/blog\/insights-updates\/health-information-exchange-work\/","title":{"rendered":"Health Information Exchange: It Should Just Work"},"content":{"rendered":"\n<p>The day is not far off when any hospital discharging a patient or doctor making a referral will be able to send along needed information seamlessly, electronically, and easily from their electronic health record (EHR) to the next point of care and to patients. This means safer, better and cheaper care.<\/p>\n\n\n\n<p>ONC believes progress toward this goal will be clear and rapid, powered by:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/healthit.gov\/certification-health-it\/\">Meaningful Use<\/a> requirements<\/li>\n\n\n\n<li>Rapid development of health information exchange services by <a href=\"https:\/\/healthit.gov\/health-it-basics\/state-health-information-exchange\">State HIE<\/a> grantees, regional networks and EHR vendors<\/li>\n\n\n\n<li>New payment models urgently requiring better care coordination and patient engagement tools<\/li>\n<\/ul>\n\n\n\n<p>It\u2019s tremendously exciting that the requisite standards building blocks\u2014a single national standard for a care summary that can be shared at transitions and transport standards to send and receive information\u2014are available, are in use, and have been included in the proposed <a href=\"https:\/\/healthit.gov\/promoting-interoperability\/-stage-2\">Stage 2<\/a> standards and certification rule.<\/p>\n\n\n\n<p>But there\u2019s a lot more we need to do to make health information exchange \u201cjust work\u201d from the standpoint of clinicians, vendors, and patients. Over the next few months, we will focus on a few key initiatives that aim to lower the cost, complexity, and difficulty of exchange so that information really can follow patients wherever and whenever they seek care.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-increasing-trust-through-common-policies\"><strong>Increasing Trust through Common Policies<\/strong><\/h2>\n\n\n\n<p>ONC has recently released <a href=\"https:\/\/statehieresources.org\/wp-content\/uploads\/2012\/07\/State-HIE-Implementation-Guidelines-for-Direct-Security-and-Trust_7-2012.pdf\">guidance<\/a> defining common policies, standards, and implementation approaches (including on issues like issuing digital certificates and identity proofing end-users) for the <a href=\"https:\/\/healthit.gov\/\/buzz-blog\/state-hie\/the-direct-project-adoption\/\">more than 40 states launching DIRECT services<\/a>. This guidance will give providers and other stakeholders confidence that DIRECT is being implemented according to technical specifications and with appropriate policies to support widespread exchange, trust, and interoperability.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-making-referrals-easy-for-providers\"><strong>Making Referrals Easy for Providers<\/strong><\/h2>\n\n\n\n<p>Providers face at least two challenges when it comes to sending patient referral information easily and electronically. They often need to leave their EHR workflow to access health information exchange services and can\u2019t exchange information with providers using different health IT products or platforms. ONC\u2019s <a href=\"https:\/\/statehieresources.wikispaces.com\/360X+Project\">360X Project<\/a> aims to address both issues through a community-led initiative to support \u201cclosed loop referrals\u201d among health care providers. The goal is straightforward but bold: Providers should be able to send referrals from their EHR workflow to providers in unaffiliated organizations, using different EHRs and supported by different health information exchange services. Community participants will develop common implementation guidance and launch pilots by the end of the year. We invite you to participate in this exciting effort, <a href=\"https:\/\/statehieresources.wikispaces.com\/360X+Participants\">joining many of the nation\u2019s leading health IT vendors<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-getting-lab-results-moving-to-providers\"><strong>Getting Lab Results Moving to Providers<\/strong><\/h2>\n\n\n\n<p>Five states and one territory in the State HIE community\u2014West Virginia, Florida, Alaska, Hawaii, North Carolina, and Guam\u2014kicked-off an exciting initiative to jump start electronic lab result reporting using DIRECT, relying on <a href=\"https:\/\/wiki.directproject.org\/file\/detail\/Implementation+Guide+for+Delivery+Notification+in+Direct+v1.0.pdf\">guidance<\/a> recently released by ONC. At our summit in May, grantees and their pilot partners put their heads together with industry experts to develop solutions that can be replicated across the nation. While many providers already receive lab results electronically and in structured form from their labs, this effort gives labs and providers, and especially those with fewer resources and capacity, another option for lab results delivery. Having structured lab results in EHRs supports chronic care improvement, quality measurement, and prompt and accurate diagnosis.<\/p>\n\n\n\n<p>As we\u2019ve said before, <a href=\"https:\/\/healthit.gov\/\/buzz-blog\/state-hie\/health-information-exchange\/\">we expect health information exchange to turn the corner in 2012<\/a>. We\u2019ve got the standards building blocks. There is an urgent need to improve care coordination. Information exchange services are developing rapidly in the market. But we also have to make sure health information exchange \u201cjust works\u201d for providers, vendors, and patients. We think that\u2019s a pretty important focus for this year.
