{"id":23099,"date":"2023-08-14T09:26:56","date_gmt":"2023-08-14T13:26:56","guid":{"rendered":"https:\/\/healthit-gov.go-vip.net\/buzz-blog\/?p=23099"},"modified":"2026-01-17T21:35:33","modified_gmt":"2026-01-17T21:35:33","slug":"lantern-lighting-the-way-on-fhir-implementation","status":"publish","type":"post","link":"https:\/\/healthit.gov\/blog\/healthit-certification\/lantern-lighting-the-way-on-fhir-implementation\/","title":{"rendered":"Lantern: Lighting the Way on FHIR Implementation"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\" id=\"h-turning-evidence-into-action\"><strong>Turning Evidence into Action<\/strong><\/h2>\n\n\n\n<p>The ONC <a href=\"https:\/\/healthit.gov\/legislation-regulation\/cures-act-final-rule\/\">Cures Act Final Rule<\/a> (Cures Rule) supports patients\u2019 and providers\u2019 access to electronic health information through Health Level Seven (HL7\u00ae) Fast Healthcare Interoperability Resources (FHIR\u00ae) application programming interfaces (APIs). To ensure that secure, standardized FHIR APIs certified through the ONC Health IT Certification Program can be accessed and used \u201cwithout special effort,\u201d the Cures Rule included a requirement to ensure that app developers could readily lookup the service base URLs (i.e., \u201cFHIR endpoints\u201d) associated with enabling patient access. These FHIR endpoints make it possible for an app to \u201cknow where to go\u201d to help a patient request their health information.<\/p>\n\n\n\n<p>Since the publication of the Cures Rule, the ONC-led <a href=\"https:\/\/healthit.gov\/interoperability\/investments\/lantern-project\/\">Lantern Project<\/a> team has been hard at work building technology to monitor these APIs and their associated service base URLs. Through its work, Lantern identified possible challenges to implementing these API requirements: (1) the lack of standardization for endpoint list publication and (2) difficulty mapping endpoints to health care organizations.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-building-consensus\"><strong>Building Consensus<\/strong><\/h2>\n\n\n\n<p>ONC\u2019s <a href=\"https:\/\/healthit.gov\/legislation-regulation\/hti-rules\/hti-1-final-rule\/\">HTI-1 proposed rule<\/a>, released in April, proposes updates to the ONC Health IT Certification Program\u2019s requirements applicable for developers certifying health IT to the \u201cStandardized API for Patient and Population Services\u201d certification criterion. One proposal was to revise and standardize the service base URL publication <a href=\"https:\/\/healthit.gov\/certification-health-it\/conditions-ccg\/application-programming-interfaces\/\">requirement<\/a>. The proposal in the HTI-1 proposed rule to revise and standardize the service base URL publication requirement builds upon our consensus-driven work and several years of evidence-building. Standardization of endpoint publication was discussed at multiple public <a href=\"https:\/\/healthit.gov\/events\/\">workshops<\/a> and <a href=\"https:\/\/healthit.gov\/events\/\">meetings<\/a>, where ONC and partners agreed on <a href=\"https:\/\/healthit.gov\/blog\/healthit-certification\/shining-a-light-on-fhir-implementation-progress-toward-publishing-fhir-endpoints\">possible solutions<\/a> that evolved into ONC <a href=\"https:\/\/healthit.gov\/certification-health-it\/conditions-ccg\/application-programming-interfaces\/\">recommendations<\/a> and ultimately, ONC\u2019s proposed update.<\/p>\n\n\n\n<p>Additionally, we are excited to see industry-led activity such as the <a href=\"https:\/\/build.fhir.org\/ig\/HL7\/smart-app-launch\/branches\/pab\/brands.html\">Patient-Access Brands and Endpoints<\/a> specification, which aims to not only provide standardized publication of endpoints and organization mappings, but also provides robust user experience when connecting patients to their health records. We eagerly look forward to the next phase of this activity.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-current-state\"><strong>The Current State<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\" id=\"h-endpoint-list-publication\">Endpoint List Publication<\/h3>\n\n\n\n<p>As of the end of 2022, we discovered through the public Certified Health IT Product List (CHPL) about 200 unique endpoint lists across all certified API modules.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>40% of the endpoint lists included production endpoints \u2014 with 1 in 4 of those published as a bundle of FHIR Endpoint resources.<\/li>\n\n\n\n<li>The remaining endpoint lists (60%) included no production endpoints (50%) or were published in non-machine-readable formats (10%).<\/li>\n<\/ul>\n\n\n\n<p>These results are similar to what we saw in the first half of 2023 and confirmed our early predictions that publication would not be uniform across certified API developers.<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-medium-font-size\" id=\"h-mapping-endpoints-to-organizations\">Mapping Endpoints to Organizations<\/h3>\n\n\n\n<p>In addition to addressing the lack of a common framework around endpoint publication, there is also a need to improve mapping of endpoints to organizations. We found that relying only on the name of the organization is insufficient and inefficient for matching endpoints to organizations, especially when it comes to health care systems that service many providers. To enable broad patient access to their health information, app developers need to tie health care providers and provider organizations to their certified health IT, if not their physical endpoints.<\/p>\n\n\n\n<p>An endpoint without specific organization information limits how the app can connect the patient to their electronic medical record since, ultimately, the patient knows their doctor, not the doctor\u2019s technology stack. An analysis of the endpoints currently ingested into Lantern shows that only about 1 in 4 lists provide the organization information about their technology users. More uniformity and richer detail can make it easier to connect patients to their health information managed by their specific health care provider.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-s-next\"><strong>What\u2019s Next?<\/strong><\/h2>\n\n\n\n<p>To learn more about this initiative, check out the L<a href=\"https:\/\/healthit.gov\/wp-content\/uploads\/2023\/08\/20230808_Lantern-lighting-the-way-FHIR-implementation-report-508.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">ighting the Way for FHIR API Implementation report<\/a>.<\/p>\n\n\n\n<p>Going forward, we plan to expand the Lantern Project and continue to integrate additional data as they become available while striving towards the goal of making FHIR APIs usable \u201cwithout special effort\u201d.
