{"id":20501,"date":"2022-06-15T13:39:33","date_gmt":"2022-06-15T17:39:33","guid":{"rendered":"https:\/\/healthit-gov.go-vip.net\/buzz-blog\/?p=20501"},"modified":"2026-01-17T21:36:15","modified_gmt":"2026-01-17T21:36:15","slug":"getting-the-best-out-of-algorithms-in-health-care","status":"publish","type":"post","link":"https:\/\/healthit.gov\/blog\/ai-ml\/getting-the-best-out-of-algorithms-in-health-care\/","title":{"rendered":"Getting the Best out of Algorithms in Health Care"},"content":{"rendered":"\n<p>The same basic technology that can predict what movie you might want to watch, what song you might want to listen to, or what item you might want to buy online, can also predict the onset of diseases, forecast costs of care, and recommend treatment options for your doctors, nurses, and pharmacists.<\/p>\n\n\n\n<p>Algorithms are not new to health care. Indeed, an important and long-standing part of our <a href=\"https:\/\/healthit.gov\/certification-health-it\/\">Health Information Technology (Health IT) Certification Program<\/a> has included <a href=\"https:\/\/healthit.gov\/test-method\/clinical-decision-support-cds\/\">Clinical Decision Support (CDS) certification criterion<\/a> covering \u201crules-based\u201d algorithms and software systems that use rules derived by experts, such as clinical guidelines, to turn \u201cinputs\u201d into an \u201coutput.\u201d While decision support technologies have existed for decades (and will be the subject of a forthcoming blog series), the application of artificial intelligence (AI) and machine learning (ML) to decision-making has been steadily growing. So too has the scale and volume of the data used to power these new computing technologies. Not a day goes by where we don\u2019t read about a new study, product, or application of AI\/ML to aid decision-making in health care settings, such as <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35258444\/\">predicting pneumonia in the emergency department<\/a>, <a href=\"https:\/\/www.nature.com\/articles\/s41746-018-0029-1%22\">predicting inpatient mortality<\/a>, and <a href=\"https:\/\/www.thelancet.com\/journals\/landig\/article\/PIIS2589-7500(22)00004-8\/fulltext\">assisting clinicians in diagnosing hip fractures,<\/a> just to name a few.<\/p>\n\n\n\n<div class=\"buzztable go-right half-width\">\n<table><caption style=\"padding-bottom: 0px\">\n<div class=\"figure\">Figure 1<\/div>\n<p style=\"margin-bottom: 0px\">As we dive deeper into this space, we find it helpful to be clear about key terms. Areas that we intend to explore further include the following:<\/p>\n<\/caption>\n<thead>\n<tr>\n<th width=\"160px\">Term<\/th>\n<th>Description<\/th>\n<\/tr>\n<\/thead>\n<tbody>\n<tr>\n<td><strong>Predictive decision support<\/strong> (<em>Model<\/em>)<\/td>\n<td>Technology intended to support decision-making based on algorithms that derive relationships from training or example data and then are used to produce an output or outputs.<\/td>\n<\/tr>\n<tr>\n<td><strong>Transparency<\/strong><\/td>\n<td>Sufficient information provided on the model, including input data, validation of performance, and&nbsp;intended&nbsp;use.<\/td>\n<\/tr>\n<tr>\n<td><strong>Trustworthiness<\/strong><\/td>\n<td>Model risks identified, mitigated, managed, and evaluated to provide confidence in the positive impact of using the model, and information about steps taken to govern the model and address negative impacts and\/or reduce bias or harm are documented.<\/td>\n<\/tr>\n<tr>\n<td><strong>Fair<\/strong> <em>(Unbiased, Equitable)<\/em><\/td>\n<td>Model&nbsp;does not exhibit prejudice or favoritism toward an individual or group based on their inherent&nbsp;or acquired characteristics. The impact of using the model is similar across same or different populations or groups.<\/td>\n<\/tr>\n<tr>\n<td><strong>Appropriate<\/strong><\/td>\n<td>Model is well&nbsp;matched to specific contexts and populations to which it is&nbsp;applied.<\/td>\n<\/tr>\n<tr>\n<td><strong>Valid<\/strong><\/td>\n<td>Model has been shown to estimate targeted values accurately and as expected in both internal&nbsp;and external data.<\/td>\n<\/tr>\n<tr>\n<td><strong>Effective<\/strong><\/td>\n<td>Model has demonstrated benefit in real-world conditions.<\/td>\n<\/tr>\n<tr>\n<td><strong>Safe<\/strong><\/td>\n<td>Model is free from any unacceptable risks and for which the probable benefits outweigh any probable risk.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/div>\n\n\n\n<p>These technologies have the potential to drive innovation, increase market competition, and vastly improve care for patients and populations. However, like any new health IT, it requires examination and inquiry to establish an evidence-base of benefits and risks. What we know from studies to date is that AI\/ML-driven predictive technology may positively or negatively impact patient safety, introduce or propagate bias, and result in increased or reduced costs. In short, results have been mixed. But the interest\u2014and potential benefit\u2014remains high.<\/p>\n\n\n\n<p>In order to motivate deeper discussion about the appropriate use of these new and evolving technologies, the Office of the National Coordinator for Health Information Technology (ONC), within the U.S. Department of Health and Human Services (HHS), has been convening stakeholders working with AI\/ML technologies through public events such as our <a href=\"https:\/\/healthit.gov\/\/news\/events\/onc-artificial-intelligence-showcase-seizing-opportunities-and-managing-risks-use-ai\">AI showcase<\/a>, a Health Information Technology Advisory Committee <a href=\"https:\/\/healthit.gov\/hitac\/\/events\/health-it-advisory-committee-43\">hearing<\/a> on algorithmic bias, and session on equity and algorithms at our <a href=\"https:\/\/healthit.gov\/\/news\/events\/2022-onc-virtual-annual-meeting\">annual meeting<\/a>. ONC is also actively monitoring and engaged in health and AI-focused initiatives such as the <a href=\"https:\/\/www.coalitionforhealthai.org\/\">Coalition for Health AI<\/a> and in a number of cross-agency coordination efforts with our federal partners.<br>We are on the cusp of what will be a long-term trend of combining health data with complex predictive models and algorithms to improve diagnosis, treatment, and care management of patients and populations. ONC-certified health IT increasingly serves as the wellspring for algorithms, providing data and tools for the creation of algorithms in health care, and as the delivery mechanism of algorithms, inserting algorithmic results and outputs into clinical and administrative decision-making. Facilitating the use of these emerging technologies in ways that maximize their benefits and minimize their negative unintended consequences is thus of high importance to ONC and HHS.<\/p>\n\n\n\n<p>&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-stay-tuned-more-to-come\"><strong>Stay Tuned, More to Come<\/strong><\/h2>\n\n\n\n<p>This is the first in a series of blog posts where we\u2019ll discuss the current and future state of algorithms and emerging technologies used to aid decision-making in health care.
