To increase the availability of rich\u00a0data sets, ONC\u2019s projects\u00a0developed methodologies\u00a0for collecting and exchanging\u00a0diverse data types\u00a0using\u00a0standardized APIs.\u00a0\u00a0<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">These projects also&nbsp;<\/span><span data-contrast=\"auto\">highlighted data quality issues such&nbsp;<\/span><span data-contrast=\"auto\">as accuracy, consistency, completeness, and usefulness. In particular<\/span><span data-contrast=\"auto\">,<\/span><span data-contrast=\"auto\">&nbsp;<\/span><span data-contrast=\"auto\">these projects<\/span><span data-contrast=\"auto\">&nbsp;<\/span><span data-contrast=\"auto\">highlighted&nbsp;<\/span><span data-contrast=\"auto\">a trade<\/span><span data-contrast=\"auto\">&#8211;<\/span><span data-contrast=\"auto\">off between richness of dat<\/span><span data-contrast=\"auto\">a and simplicity of standards.&nbsp;<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">Standardizing data generally results in improved data quality and consistency, but the use of data standards does not always result in data standardization due to inconsistent implementation of current standards and the use of proprietary standards, creating a&nbsp;<\/span><span data-contrast=\"auto\">constant&nbsp;<\/span><span data-contrast=\"auto\">need for harmonization.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-robust-infrastructure\"><b><span data-contrast=\"auto\">Robust\u00a0Infrastructure<\/span><\/b><\/h2>\n\n\n\n<p><span data-contrast=\"auto\">Precision medicine requires robust health IT infrastructure and additional functionality may be needed to capture, access, and share data in an interoperable manner.&nbsp;<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">The projects<\/span><span data-contrast=\"auto\">&nbsp;focused on building<\/span><span data-contrast=\"auto\">&nbsp;functionality&nbsp;<\/span><span data-contrast=\"auto\">that allowed participants<\/span><span data-contrast=\"auto\">&nbsp;to capture data<\/span><span data-contrast=\"auto\">&nbsp;from outside clinical care,<\/span><span data-contrast=\"auto\">&nbsp;access data from other informatio<\/span><span data-contrast=\"auto\">n systems,<\/span><span data-contrast=\"auto\">&nbsp;and advance interoperability through&nbsp;<\/span><span data-contrast=\"auto\">the&nbsp;<\/span><span data-contrast=\"auto\">use of standardized functions<\/span><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">Projects revealed that testing and validation activities are important for successfully implementing new functionality, but these activities can often be&nbsp;<\/span><span data-contrast=\"auto\">resource-intensive<\/span><span data-contrast=\"auto\">. The projects also revealed other infrastructure considerations and challenges. For example, firewalls&nbsp;<\/span><span data-contrast=\"auto\">impede<\/span><span data-contrast=\"auto\">d<\/span><span data-contrast=\"auto\">&nbsp;access to data sources such as devices that reside<\/span><span data-contrast=\"auto\">d<\/span><span data-contrast=\"auto\">&nbsp;outside the firewall, and some sites did not have access to adequate data storage and management resources.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-clinical-integration\"><b><span data-contrast=\"auto\">Clinical Integration<\/span><\/b><\/h2>\n\n\n\n<p><span data-contrast=\"auto\">Several projects tested the ability to integrate precision medicine data into clinical systems. The projects reinforced the importance of clinical workflow consideration but&nbsp;<\/span><span data-contrast=\"auto\">also&nbsp;<\/span><span data-contrast=\"auto\">the&nbsp;<\/span><span data-contrast=\"auto\">need&nbsp;<\/span><span data-contrast=\"auto\">to augment data with educational materials to understand and communicate genomic information.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">Genetic test results shared in PDF format were not easily integrated into an EHR. Clinical and laboratory information systems had difficulty accommodating complex genomic use cases, forcing development of customized implementation guides which&nbsp;<\/span><span data-contrast=\"auto\">limit<\/span><span data-contrast=\"auto\">ed<\/span><span data-contrast=\"auto\">&nbsp;the guides\u2019 utility across provider organizations.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">The projects&nbsp;<\/span><span data-contrast=\"auto\">also&nbsp;<\/span><span data-contrast=\"auto\">revealed the importance of using standards that are device or wearable agnostic, which facilitates integrating data from such devices into clinical care<\/span><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-policy-development\"><b><span data-contrast=\"auto\">Policy<\/span><\/b><span data-contrast=\"auto\">\u00a0Development\u00a0<\/span><\/h2>\n\n\n\n<p><span data-contrast=\"auto\">As data are collected from individuals, there are&nbsp;<\/span><span data-contrast=\"auto\">new&nbsp;<\/span><span data-contrast=\"auto\">policy considerations for privacy and security, ethics, and data s<\/span><span data-contrast=\"auto\">haring.&nbsp;<\/span><span data-contrast=\"auto\">If highly identifiable patient information would be shared with different clinical entities and researchers, this may create privacy risks.&nbsp;<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">Even though projects used<\/span><span data-contrast=\"auto\">&nbsp;<\/span><span data-contrast=\"auto\">existing<\/span><span data-contrast=\"auto\">&nbsp;standards<\/span><span data-contrast=\"auto\">,<\/span><span data-contrast=\"auto\">&nbsp;<\/span><span data-contrast=\"auto\">authenticating<\/span><span data-contrast=\"auto\">&nbsp;users and devices across multiple settings remains a challenge<\/span><span data-contrast=\"auto\">. A<\/span><span data-contrast=\"auto\">lign<\/span><span data-contrast=\"auto\">ing<\/span><span data-contrast=\"auto\">&nbsp;security and permission<\/span><span data-contrast=\"auto\">s<\/span><span data-contrast=\"auto\">&nbsp;across&nbsp;<\/span><span data-contrast=\"auto\">disparate&nbsp;<\/span><span data-contrast=\"auto\">systems often resulted in<\/span><span data-contrast=\"auto\">&nbsp;time-consuming<\/span><span data-contrast=\"auto\">&nbsp;connection problems.<\/span><span data-contrast=\"auto\">&nbsp;<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">Projects also identified concerns&nbsp;<\/span><span data-contrast=\"auto\">about&nbsp;<\/span><span data-contrast=\"auto\">industry-wide use of access control and security standards, as well as the need for clarity&nbsp;<\/span><span data-contrast=\"auto\">on&nbsp;<\/span><span data-contrast=\"auto\">how genomic data<\/span><span data-contrast=\"auto\">&nbsp;<\/span><span data-contrast=\"auto\">are protected under current policies and legislation<\/span><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-continuing-progress-nbsp\"><b><span data-contrast=\"auto\">Continuing Progress<\/span><\/b><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/h2>\n\n\n\n<p><span data-contrast=\"auto\">ONC\u2019s projects have begun to address health IT precision medicine needs.&nbsp; However, gaps remain in routinely collecting diverse data types and anticipating future data needs; quality and completeness of collected data; broad use of relevant standards; use of tools for implementation testing and validation; and use of privacy preserving techniques.<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:200,&quot;335559740&quot;:276}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span data-contrast=\"auto\">ONC<\/span><span data-contrast=\"auto\">&nbsp;is advancing high-priority&nbsp;<\/span><span data-contrast=\"auto\">health data&nbsp;<\/span><span data-contrast=\"auto\">standards&nbsp;<\/span><span data-contrast=\"auto\">and developing a better understanding of the current API and app ecosystem.&nbsp;<\/span><span data-contrast=\"auto\">ONC\u2019s&nbsp;<\/span><span data-contrast=\"auto\">21<\/span><span data-contrast=\"auto\">st<\/span><span data-contrast=\"auto\">&nbsp;Century Cures Act Final Rule requires the adoption of the&nbsp;<\/span><a href=\"https:\/\/healthit.gov\/\/isa\/united-states-core-data-interoperability-uscdi\"><span data-contrast=\"none\">United States Core Data for Interoperability<\/span><\/a><span data-contrast=\"auto\">&nbsp;(USCDI) standard, which will help improve standardized data sharing needed for precision medicine.
